This is in a TC pmax?

Maybe an IPI (intelligent peripheral interface) TC controller board?
IPI was a late-80s higher-end alternative to SCSI, before fast/wide
SCSI took off.  SGI server boxes like the 4D/3<n>0 (with <n> R3000
CPUs) used them.

I've also seem some weirdo SCSI devices that used three-row D-shell
connectors (some old Exabyte units, iirc).

Your best bet may be to try and find the disks that came with the controller.
