It's now been quite a few months since I eyeballed the guts of a
MAXine, but aren't the PROMs on the CPU board?

Unlike the R4400 (5000/260) board, the R4000 daughterboard for the
5000/150 or 5000/50 has a pair of DIP switches on it.  The settings
required for the two systems are different, but I don't recall which
you need for a MAXine.

Could this be your problem?
