Yeah, it's a Rage Pro which belongs to the Mach64 family. ( /not/ a Rage
128 but since it's only used as an identifier it doesn't really matter )

Try to use the 'atimisc' driver - I don't see a message about it being
loaded in your config. In theory the 'ati' driver should handle it
though.
