I don't know about the 6500, but I had no luck booting from anything but
floppy or internal hard disk on the PowerBook 3400 with OF 2.0.1.

Ouch.  My only suggestion is to try a thorough cleaning of the floppy
drive with new floppies, writing the floppies on the same drive that
you'll be reading from.  This (finally) worked for me on my PB3400.
