The situation seems to be what you most hate (sorry).
It looks code is loaded onto 0x04000 and location 0x4090 seems
to have problem ( or nothing loaded there).

Machine:
StarMax 3000/240 OF 2.0.2
I have 40Mbyte scsi disk connected to ID=6 and
sometimes use it for boot.fs.

(1) boot.fs boots fine:
root@harry  16:28:37/020516(~)# dd if=/a/j/NetBSD/arch/macppc/snapshot/2002040-
1.5ZC/installation/boot.fs of=/dev/rsd0c bs=64k
