Inside Macintosh will have some of this information.  The old IM:V
covered the older macs--I don't know if the PB1400 and other late-model,
pre-OFW systems follow the same method, but I expect it's not much
different.

There is a late draft of the standard somewhere around
playground.sun.com, I think, but it's been a while since
I dug around for it.  Were you thinking of trying to
provide an OFW-like interface for NetBSD to use, or just
trying to understand the existing system, or what?

As someone mentioned, there's Tenon's MachTen, but it's not free.  Apple
did release MPW, but it's cli is pretty funky, IIRC, and not terribly
general purpose.
