Please try 19991023 version of GENERIC_MD.  It booted on
iMac for me.

Did you press the keys other than shift first?

When USB keyboard hung, I pulled and reconnected the cable
then press the shift key several times and I was able to use
the keyboard.

USB problem are in 19991023 binary sets.  Workaround above
did for me.  I don't have USB model myself (I had an iMac on
lone), so anyone who have them will help further...
