Quantum Fireball TM 1280?  I had a similar experience when installing on
one of those.  Unlike you, I was using different root and usr partitions
(IIRC 100 meg swap, 100 meg root, 50 megs mac OS, rest usr).

The way I finally made it happy was in putting the root partition first
(followed by usr, swap, macOS last).

If you're using Apple's Drive Setup for formatting, "low level format"
and "zero all data" are probably preferable options, although they will
make the process take a little longer.
