What version of the kernel are you running? (You say "next-to-latest",
but I'm not sure what that means :) 1.4.1? 1.4W?) But since your
/etc/ttys came with ttyE0 rather than ttye0, I'll assume you're running
NetBSD-current, i.e., 1.4[UVWX] or so. When the kernel boots, do you get
a message like "wsdisplay0 at macfb0: console (std, vt100 emulation)"?
If so, ttyE0 is correct. Here's the line from my /etc/ttys:

ttyE0 "/usr/libexec/getty Pc" vt220 on secure

The "can't exec getty" error sounds more like it can't find
/usr/libexec/getty for whatever reason. Doesn't that message come after
the "setting tty flags" line? That is, if you change ttyE0 to ttye0, it
hangs _earlier_?

Boot into single-user mode, do a "mount -r /usr", and let me know what
"ls -lo /dev/ttyE0 /usr/libexec/getty" shows. It should be something
similar to:

crw------- 1 root wheel - 46, 0 Apr 6 01:44 /dev/ttyE0
-r-xr-xr-x 2 root wheel - 24576 Apr 4 14:27 /usr/libexec/getty*

The dates may be different, and maybe getty's filesize too. /dev/ttyE0
should be "46, 0" though.
