That'll work with NetBSD, but I think it will only drive a 13" monitor.

These work great with NetBSD.

That's the standard Apple video connector.

This sounds like the PC compatible cards that came installed in the 
Power Mac 6100 PC Compatible.  I've got two in a drawer.  It's a 486 
PC on a card that only runs DOS and Windows, since it needs some 
special code to interface with the Mac.  It's no longer supported by 
Apple, so you have to run something like Mac OS 8 (or maybe even 
earlier) on the Mac side to get it to work.
