Well, from the Slots output you posted, this is a card that obeys the
Cabletron software interface.  As near as I can tell from the source,
we don't support anything with the DrSw for Cabletron.  In fact, I'm
kind of surprised that we tried to attach it at all!

	Category = 0x0004 --> NUBUS_CATEGORY_NETWORK
	Type     = 0x0001 --> NUBUS_TYPE_ETHERNET
	DrSw     = 0x0001 --> NUBUS_DRSW_CABLETRON
	DrHw     = 0x010f --> NUBUS_DRHW_ASANTE_LC

In order to supoprt this card, you'll need to find out for sure that it
is a Sonic or 8390 chip-set, and you'll need to find out where in the
card's address space the control registers are located. You'll also need
to find out where to get the card's Burned-In Address...  And that's
just for starters!
