Yes, since option-{1,2,3} (there was no control) broke things for many
international users, this feature was changed to a config option.  I
believe that adding:

options 	ALTXBUTTONS

to your kernel config file and recompiling will enable the desired
behavior.

Hmmmm...it should "just work" if it actually follows extended mouse
protocol.  If you want to see how it works, tho, grab a copy of ADB Parser
off of Apple's developer ftp site and take a look at what kind of data the
mouse sends when you press the buttons.  BTW, what does the 2nd button do,
anything at all?
