This has always been the case for my IIci running 7.0.1* of MacOS.
I can always boot if I turn on the computer with SHIFT held down
to boot with no extensions. I have _never_ been able to boot NetBSD
if I did not hold down the shift key during MacOs boot. I get the
same behaviour you report. I have
tried removing all extensions and virtually all fonts from my system
and still the only way to boot NetBSD is hold down SHIFT. Why? I don't
know, but you may have luck doing the same.
