The documentation states to add:

xmodmap .xmodmaprc

to your .xinitrc file in your home directory.  You probably want to have at
least:

xmodmap .xmodmaprc
xterm &
twm

in the file, otherwise X will start and then terminate.

I installed X for the first time a couple weeks ago.  Maybe someone could
update the document on puma.
