Ok so that fixed the first problem...

I suspect you've created a non-standard layout....
The code tries quite hard to locate /boot, but you are allowed to shoot
yourself in the foot :-)

Can you send the layout part of the output of 'fdisk wd0' and 'disklabel wd0' ?
