Any idea what this means?

I was writing to an SD card at the time using
        - PQI "travel flash - USB 2.0 All in onje card reader"
        - PQI 256MByte SD card
        - netbsd 1.6.1 (just installed this morning).

It has done it twice now, locked up during a file copy,
the whole machine freezes when I pull the device off the USB bus.
The device was formatted on a Palm (Zire71), fsck'ed & mounted fine.
