That did it!  I poked around /sys, read the comment at the beginning of
/sys/dev/isa/i82365_isa.c, and used:

options	PCIC_ISA_INTR_ALLOC_MASK=0x0e10
options	PCIC_ISA_ALLOC_IOBASE=0x400
options	PCIC_ISA_ALLOC_IOSIZE=0xbff

(I set PCIC_ISA_INTR_ALLOC_MASK as "restrictively" as possible.)

I will investigate submitting what I found to the FAQ in that this should work
for a ThinkPad 560E.

I use the following in my TP 560; it works great.  The settings in the FAQ
didn't work for me either.

options         PCIC_ISA_INTR_ALLOC_MASK=0x7f7f
options         PCIC_ISA_ALLOC_IOBASE=0x400
options         PCIC_ISA_ALLOC_IOSIZE=0xbff
