I got a tip from several people (thanks!) to run SOFTSET and enable 32 K
iomem starting at 0xc0000, 0xc8000, 0xd0000 or 0xd8000. This made the
card probe, but it also crashed the kernel (1.3 through 1.3H) badly. As
in the screen going completely blank and the machine having to be hard
reset.

I quickly ran out of patience and bought a cheapo NE2000 clone instead
which worked out of the box.
