I've had this problem through several versions of the system.  I filed a PR
(kern/5026) and have various kernels/core files at
http://www.egr.unlv.edu/~greg/netbsd but I don't think anyone has found the
time to investigate it.  I've dont some investigation, but my understaning
of the vfs subsystem is apparently not good enough for me to find the
problem.
