Not hard at all, if you have your DNS set up properly (in the outside
world).  I do this on a constant basis using ETRN on my ISP's sendmail.
If your ISP runs sendmail v8, ETRN should be available.

This does require having a local DNS running as well, but that's
easily enough done.
