Now I'm stumped.  Matt Jacob sent me a Qlogic board to try.  It works
fine; SRM sees it.

It is almost totally identical to the Qlogic board I was trying that didn't
work.

Mine:			His:
Qlogic			Qlogic
ISP1040B 2405101	ISP 1040B 2405101
MIQL42/N0105D		MIKL8U/N9750D

Both have 60Mhz crystals, and a chip with a sticker on it saying "AA67A".

Mine says "PC2010403-09 K", his says "PC2010403-11-E", in the "assembly/rev"
section.

With his, "show config" says
     Bus 00  Slot 09: QLogic ISP1020 Scsi Controller
                                   pka0.7.0.9.0          SCSI Bus ID 7
                                   dka0.0.0.9.0           SEAGATE ST34501W
With mine, it says:
     Bus 00  Slot 09: QLogic ISP1020 Scsi Controller
