One source worth trying might be David Turner at Island Computers
(www.islandco.com). He was selling a bunch of PC164LX boards a while
back (probably still has some) and seems to have figured out several
sources of working aftermarket SCSI cards.

I'm surprised the Qlogic 1040B doesn't work, as that's the "same" card
as the DEC KZPBA, which is supported by SRM/VMS/Tru64 (I know that
information doesn't necessarily transfer to the PC164LX board, of
course). Are you using the latest firmware on the PC164LX?
