Power-up reset is known to be a very hard problem.  Is there a power-good 
signal on the cable from the power supply?  That's the right solution. 


If you want suggestions for switch debouncing, I suggest looking 
at page 507 in "The Art of Electronics" by Horowitz and Hill.  It's 
a big expensive book, but anybody into this sort of thing should 
have a copy.
