The only current source for the Qlogic SCSI cards that I can find, who's
central chip (Qlogic ISP) is really all (except some random NCR cards) that
the SRM understands right now for disk booting, is a company called EBV
Electronics. Qlogic no longer sells the cards retail.

EBV Electronics in Sunnyvale, CA (talk to Annette @ 408 522 9599)
will sell the ISP 1040 for about 205$, which is a definite bargain against
the Adaptec 2940UW (which is over 300$ for the equivalent Ultra Wide
functionality).

I'll report back about my experiences with the card, but my take on
it is that it's really too bad they dropped out of the retail market.
