Okay, with Jeremy Reed's advice about
printing to *.ps via Mozilla, then a
bit of hand tweaking of the *.ps before
doing ps2pdf, I get this...

...which isn't yet done but still not
half bad even so.

Thanks for everyone's help. All I need
to do now is write an XSLT for pre-composing
the pdfmarks so I can just cut-n-paste
into the *.ps at the prolog.
