AHH! Got it. Revision 1.21 to
/usr/src/distrib/i386/floppies/fdset-common/Makefile.inc needs to be
uploaded to the 1.5 branch (it was only committed to current). That
fixes my problem.
Concerning other matters:
18k vs. 38b: either works -- I did both
INSTALL variant (under 1.44M): works
