What kind of system is this on?  From the "fdisk" notes, I assume it's a PC.

OK.  Good first step.  Always nice to know that the controller sees the
drive.  I think the tool you're looking for is 'disklabel' -- use it
with care.  After disklabel, you'll want 'newfs'.
