 this may have been caused by misconfigured npf which did not correctly add IP addresses to blacklist database, therefore it encountered what blacklistd manual says or an unknown race condition with a prerequisite that the source address is connecting with many threads. probability to encounter this condition should increase if nfail is set to a larger number.
 
 was not able to reproduce on current, but did not test much.
