 Assuming the problem isn't that the card is bad (which is not
 impossible even with a new one, but seems unlikely) ... one other
 thing to check is whether you've got enough power. I can't remember
 if this is regularly an issue with the rpi or not, but I figure I
 might as well mention it since nobody else has said anything at all. :-/
