Some Dell flat panel monitors contain a USB hub that allows USB peripherals to be connected to the monitor (in this case a USB mouse).
This is not initializing. From dmesg:

uhub5 at uhub4 port 1
uhub5: NEC Corporation USB2.0 Hub Controller, class 9/0, rev 2.00/1.00, addr 2
uhub5: 4 ports with 4 removable, self powered
uhub5: cannot open interrupt pipe

This was working as of a couple of weeks ago, so looks like a recent
modification.
