The packages come with teTeX which includes pdfTeX 3.14159-13d.  That version of TeX has problems including .pdf images generated by programs like ps2pef12.

Fortunatly there is a new version of pdfTeX around ...-14* series.

Now for the wierd bit: the pdfTeX mailing lists refer to the new version (and the bugs in the old).  I've used the new version (confirming the fix on Red Hat GNU/Linux; but I've never managed to track down the source.
