What is the correct default spool directory?
src/usr.sbin/lpr/common_source/pathnames.h, line 41 contains:
#define _PATH_DEFSPOOL "/var/spool/output/lpd"
src/share/man/man5/printcap.5, line 121 contains:
.It "sd str" Ta Pa /var/spool/lpd Ta No "spool directory"
and src/etc/mtree/NetBSD.dist creates /var/spool/lpd
and /var/spool/output but not /var/spool/output/lpd in the default tree

If you don't specify a spool directory in /etc/printcap,
then the lpr tools will attempt to spool in /var/spool/output/lpd
That directory does not exist in the default tree:
$ lpq
lpq: lp: cannot chdir to spooling directory
