The reset recovery code for the ISP HBA adapter doesn't work very well.
>How-To-Repeat:
        Jam up the ISP by powering off a disk on the ISP's SCSI bus while
        that disk is really active. Eventually a command will time out.
        The ISP driver will reset, but not cleanly or completely, sometimes
        leading to a panic:
