Could be the disk but the error reported is interface CRC error which 
could mean a cable fault of some kind as well. Its something that has 
happened to me in the past and replacing the cable fixed it for me.

So check that the cable has not come loose at either the drive or 
motherboard end. Its also worth considering a cable swap if you have an 
easily available spare.

It could also be a connector failure on the motherboard or disk as well 
which would be slightly harder to deal with.
