Oh, for the record:

This appears to have been the problem.  Prior to updating the
kernel, two consecutive attempts ran into problems with the same
spot in the filesystem.

After updating the kernel, the problem has gone away.

(In retrospect, I didn't try rebooting with the old kernel before putting
in the new...  Oh well.)
