...Just to touch on the obvious, have you checked your disklabel and
made sure you don't have any overlap between root and some other
filesystem?   Check both in-core and on-disk disklabels, maybe something
is zorching the disklabel in some mysterious way.

The other obvious thing is that something is skrogging the raw device.

Neither of the above may be the case, but they're worth looking into.
