I too have that book. Let me know if you find something which covers later
revisions well. :-)

Technically it's how the board maker wired the IDSEL lines. There's a
description around p322. Also see page 522 for how the VL82C59x handles
IDSEL (which is different then the spec's example quoted on p322!).
