Get rid of the atf-c++ formats module

Put every file parser where it belongs: next to the (only) code that uses
it.  This is to remove a too-coupled module that provides no real benefit
to external users.  It also lets us remove the tests/atf/formats subtree,
which will benefit ticket #11.
