No, the MPI350 is device ID 0xA504.

It's labeled as a Cisco Aironet 350.  AIR-PCI352 is the part number.  It
is the full size PCI card version.

It turns out that the card was shipped with corrupt firmware.  Flashing
it from another machine solved my problems.
