Clarifications and fixes:

- (Trailing) white space and punctuation fixes;
- elaborate on who to manually send a PR's follow-up in the case
  GNATS doesn't handle that;
- describe GNATS' outgoing messages' e-mail header usage;
- stress that even when replying to a PR follow-up's sender directly,
  one may have to add the address of that sender to its outgoing
  reply message explicitly.
