MKCATPAGES and MANINSTALL

Hi all,
the recent problems in pkgsrc with BSD-Makefile using packages is a
consequence of two different variables being used to control whether cat
pages should be installed or not.

MKCATPAGES is used to specify whether cat pages should be built. That
was changed to "no".

MANINSTALL lists what variations are to be *installed*. If
MKCATPAGES=no, it simply skips them. This still defaults to
"maninstall catinstall htmlinstall". This is the value used by pkgsrc to
decide if cat pages should be filtered out of the PLIST or not.

Short term, MANINSTALL needs to follow MKCATPAGES. But the more
interesting question is, if both ways are still needed and desirable.
