As you mentioned, the "patched MIT pthreads" would be what I'd start
from, taking changes (all in nice patch format, gotta love pkgsrc!)
from our current base.

There are at least two patched pthreads versions out there.  I've
contacted one author, and he would love to have any changes we make
back, and perhaps even take our patches and intregrate them himself.
