I have recently bootstrapped on a fresh installation of Debian Buster

I expected to find a "/usr/pkg/var/cache" but it's not there. Has
anything changed recently? I know there have been changes to
${PREFIX}/pkgdb, but this shouldn't have interfered with
$VARBASE/cache.
AFAIK, $VARBASE/cache is not created by bootstrap but is created by a
package that require it.

On my pkgsrc installed Mac,

% ls /opt/pkg/var/cache
fontconfig

When fontconfig package was installed, it created $VARBASE/cache/fontconfig.

I know. What you say makes sense. but I am pretty sure that the cache directory had been created without my intervention in my last 3 bootstraps on Debian.
