Likewise, I don't doubt that you're seeing it fail, but I'm not sure

On what platform are you seeing this?
What version of the postgresql-server and postgresql-pltcl packages?
Does anything get logged in the data base dir's errlog?
What does `nm ${LOCALBASE}/lib/postgresql/pltcl.so' show you?
