Looks good to me now, assuming all tests pass with it.  (Do we have
any automatic tests for ffs_reload &c.?)

The lfs changes look suspect -- there is nothing interlocking with
vp->v_interlock before vget -- but I can't figure out the twisty maze
of code paths in lfs_bmapv, so it was probably wrong to begin with and
I won't worry too much about it...
