I have very similar symptoms on an amd64 nvidia/via system.
about 10% (or less?) of the time it locks up before
"boot device: wd0" (normally the usb keyboard is not
attached), if it doesn't lockup than the cflash reader
works fine. (almost always there is no cflash installed
on this system).
BTW - I also dono why wd0 is UDMA 5 while wd1 is UDMA 6,
they are the same drive.... and re0 is flaking.
This is netbsd-4-0-RELEASE can try any recommended
patches.... The dmesg is below...
