Try turning off sync negotiation in the bios if you can.  I had this
problem when I was putting in some new hardware (new CD-writer & 9gig
hard disk) my previously rock solid old hard-disks started acting up,
the new disk was getting the mode sense 0x0 on it.  I just put it down
to the dodgy scsi lash up at the time after I got it working by
turning off sync negotiation on the old HD.  Before anyone starts
thinking that it's a ncr (I have a ncr 53c810) issue - the problem was
exactly the same with an adaptec 2940.  Now that I have the old disks
off the chain I do not see the problem.
