Pull up revision 1.3 (requested by kent in ticket #494):
- add support for headphone-boost mixer controls
- fix a bug of creation of output mute
- unmute all controls by azalia_set_mixer()
- if a bidirectional pin is green, orange, gray, or black, set
it as "output" direction by default.
