There's one #define in /usr/share/misc/style.  What more documentation
need there be?  8-)

More seriously: the standard as i've always understood it has been
#define<tab>.  yes, somebody should probably note that in prose in
'style'.

I've never really been convinced that that's The Right Thing, but i've
never been convinced it was wrong, either.  (other parts of KNF,
though...  8-)
