* Remove the inclusion of <sys/param.h>; the most common use for this was
  the MAXHOSTNAMELEN constant whose semantics are not intended for network
  database lookup operations, and which pollutes name space.
* Per XSH98, provide symbols from <inttypes.h>.
* Add various bits of name space protection.
